What Causes Burning Sensation While Urinating And Urinary Retention While On Ciprofloxacin?

Question: Urologist put me on Cipro for UTI in both bladder and prostate 3 days ago. I have had severe painful burring on urinating for past 2 months. Before that I already had frequency, intermittent, urgency problems due to BPH for many years, but UTI has made symptoms much much worse in past month: need to urintae every 30 to 40 minute and incredibly painful burning. Although have been drinking copious amounts of H2O with Cipro, which I take at 8 am and 8 pm, it seems like it is causing my urine stream to virtually dry up at night so that attempts to urinate are really painful burning but only a tiny dribble is produced, and I am getting up every 30-45 minutes. In mid-day, urine flow seems relatively more plentiful. i haven't noticed any improvement in symptoms after 6 doses in 3 days. --- QUESTION: can the Cipro be decreasing the urine flow despite all the water I am drinking, and therefore make the burning even worse/

Hello
Thanks for your query ,based on the facts that you have posted it appears that you have been known to have BPH and having burning urination since very recently .

The burning that you have is due to UTI secondary to BPH which seems to be aggravated recently and poor results are likely to be due to organisms may not be sensitive to Ciprofloxacin .

consult qualified Urologist for clinical examination and get following basic tests done to trace out the organisms causing infection and their sensitivity pattern
1) Urine routine and culture.
2) Ultrasound scanning of abdomen and pelvis

In the mean while start taking broad spectrum antibiotics like Cefixime along with urinary antiseptic like Nitrofurantoin twice daily and urine alkalizer thrice daily .Later on switch on to appropriate antibiotics as per culture report.

Get the prescription of medicines from your family Physician.

Ensure to drink more water .To keep your urine dilute This will help to control dysuria.

-- My urologist did take urine specimen for culture on 2/8. I am expecting to get results today, but have not received yet.

Unfortunately, he is away until Monday, 2/15, but I am waiting for his nurse to call me back. I was told she can order Rx's if needed, so I will discuss your suggestions with her.
--FURTHER QUESTIONS: You said to get ultrasound of abdomen and pelvis. The urologist did do ultrasound of bladder, but just to see if it was emptying. (He said it is, but I typically pass so little urine -- 1 oz or less, rarely 2-3 oz -- that I wonder.)
-- QU: What would ultrasound of abdomen + pelvis reveal? What is best way for me to request it?
-- QU: Is Cefixime generally more effective for UTI than Cipro?
-- QU: Re: Nitrofurantoin: You suggested taking it as urinary antiseptic. I see Drugs.com says it is an antibiotic also, and says should not take if urinating less than usual or have urination problems. --- So, I'm confused, since with severe BPH and UTI, i am definitely urinating very little, although very frequently. As I stated originally, although I am drinking copious amounts of water (48-64 oz per day), at night, despite terrible urgency every 30-45 minutes, I get almost no urine production, even though the few drops burn terribly.
--QU: Are urine alkalyzers available OTC? Or Rx?
What brands do you suggest?

Cefixime is second generation Cephlosporine ,more effective than cipro.

Ultrasound scanning will help to reveal size of the prostate ,post void urine residue ,and whether you have retention of urine and chronic changes in bladder due to long standing bladder outlet obstruction .This will help us to decide any need for surgical intervention or catheterization

2) Cefixime is more effective than Ciprofloxacin

3) Nitrofurantoin is antibiotic and urinary antiseptic acting specifically on urinary system hence routinely prescribed .It is not advised when there is less urine production by kidneys which is not the case with you hence can be taken .

4) You can take Citralka pr Oricitral which is freely available in XXXXXXX .I have no knowledge about its availability over counter in your country .

Again, thank you so much. VERY helpful.
unfortunately, another doctor in my urologist's office (the one I see is away until Monday) said they would not consider making any changes in the antibiotic or additional meds until they have the path report on the urine specimen, which they won't have until tomorrow.

At that time, I will try to ask them again about changing to Cefixime from Cipro.
