If you would have presented to me as an outpatient,I would have told you that - commonest causes of the symptoms you are having are-
Diabetes
Vitamins deficiencies
Alcoholism
Drugs
Neuropathy
Your MRI spine and nerve studies being normal rules out spine and large nerves abnormality.
Please get the following investigations and follow up with reports-
Blood sugar fasting,vitamin B12 and Thyroid function tests.
Meanwhile,please start taking methylcobalamin supplements.PLEASE TAKE ALL MEDICINES UNDER A PHYSICIAN'S GUIDANCE.

Hi,
Thanks for being in follow-up.
I have gone through your detailed medical reports.Thyroid function tests,vitamin B12,Blood sugar and Nerve conduction studies are all normal.MRI spine has already been done and is normal(as you mentioned).So,there does not appear to be any apparent cause for your symptoms.Chronic alcohol intake,however,can be a cause, if it fits in your case.
Besides,Anxiety or psychosomatic is the likely cause of the symptoms.Please start doing mental relaxation exercises/meditation/yoga regularly and your symptoms will probably improve.
However,visiting and getting physically examined by a neurologist will be helpful,of course.There are some of the excellent neurologists at XXXXXXX You can refer to the likes of Dr.T XXXXXXX or Dr.J.Manickavasagam.
