A superimposed bacterial infection should be looked for. If there is hypoxemia, shortness of breath, persistent wheezing, fever or deranged metabolic profile then antibiotics based on severity should be initiated. If the lower respiratory tract infection is causing an exacerbation of a preexisting COPD or bronchial asthma then corticosteroids may also be added. I would advise a second opinion if the above symptomatic treatment doesn't lead to symptomatic relief within 24 hours.
