Detailed Answer:
Dear Friend.
Hi , I am Dr Anshul Varshney , I have read your query in detail , I understand your concern.
If your Echo And CT Angiography are normal, the ache may be related to:
1. Anginal pains
2. Anxiety
3. Palpitations due to any cause like Thyroid Disorder
4. Acid Peptic Disease or Gastro Esophageal Reflux Disease (GERD)
However, to give you a better answer, i need following details:
1. Since when are you suffering from this?
2. At what time do you have these episodes? Continuous? In night? In walking/exertional activity?
3. How is your sleep?
4. Any Palpitations? Anxiety? Stress?
5. Any history of Smoking or Alcohol consumption?
6. Are you overweight?
7. Any history of acidity?
8. Any history of sweating? Flushing?
9. Your Blood Pressure and Blood sugar levels?
10. Any family history of heart disease?
11. Any history of Thyroid disorder?
12. What is your Heart rate? You can check your pulse for 1 minute and then tell me.
13. Please mention if any medications you take for any problem?
This is my personal opinion based on details available here. If you still have any query you may please ask me and revert with the answers of the above questions for a better help.

Doesn't seem to be cardiac pain
Detailed Answer:
Dear Mr XXXXXXX
Your history is suggestive of Anxiety disorder.
Chest pain related to cardiac issues is not continuous. It is intermittent, increases in exertion. Moreover your Cardiac Tests are normal.
So, i don't find any reason for you to worry.
I would advise you to quit smoking completely.
Get a lipid profile done, if you have not got it done.
I advise in such conditions following that you may discuss with your doctor:
1. To take SSRI group of drugs like Escitalopram
2. To take drugs for acidity like Pantoprazole
3. To take Etizolam twice daily.
4. To go for relaxation therapies.
If you have any further query please ask me.
Stay Healthy.
