Question: Three weeks ago I was in the ER for chest pains and they call it mild pancreatitus. I have adjusted my diet, but I have had some trouble swallowing over those weeks.Tonight I have coughed up a lot of blood and two very large chunks of what appears to be bloody tissue. I have them in a glass. I feel fine otherwise. What do you think?

Forgot to mention there has been a lot of smelly belching.

Brief Answer:
Blood clot most likely from ulcer in gut.

Detailed Answer:
Hi.
Thanks for query.

Have gone through your details.
It seems that you have gastritis not pancreatitis.Pain in chest is often caused by gastritis.
Have gone through the pictures. It looks like clot of blood and most likely the source of blood are ulcers in gut.
Though modt ulcers often heal on their own but Bleeding is a warning sign.
Smelly belch is also suggestive of bleed in stomach.Blood is acted upon by a number of bacteria yeast and fungi causing production of gases like hydrogen sulphide and ammonia giving smell to belch.

Ulcer can be present any where along the lining of the oesophagus, stomach or small intestine.

H.pylori bacteria cause most ulcers. When H. pylori bacteria infect the intestine, the infection can weaken the lining of the intestine. These medicines irritate the lining of the stomach and upper intestine, making it easier for stomach acid to damage the lining. Your stomach makes too much acid hence causing sores which bleed.

suggestion-
Go for endoscopy. An endoscopy is a procedure in which your doctor uses specialized instruments to view your gut and see ulcers.Till that time.
Rx
Avoid or limit caffeinated drinks and alcohol.
Refrain from smoking, as it can slow healing.
Take sucralfate 1 gram four times a day for ulcer healing.
Take all of your antibiotics, if prescribed, for H. pylori infections(after endoscopy)
Take a proton pump inhibitor like Esomeprazole 40 mg twice daily along with a prokinetic drug (eg.Domperidone) for few days.

Follow up with report of endoscopy.
