What Causes Chronic Diarrhea And Irritable Bowel?

Question: I am working in a company which is in a city far away from my home town.
I usually take food from restaurants. It has been 1 year since i started taking food from different restaurants in the city. During the last 1 year i got diarrehea several times(more than 10 times) due to food poisoning. when i have loose stools more than 3 times, I consult a doctor and take an antibiotics course(ofloxacin 200 and orinidazole 500). After the antibiotic therapy my condition will be cured and everthing will be normal. But for the last 2 months , I have an irritable belly. whenever i take milk products , fruits like apple and spicy food , i get diarrehea. Since last two months my stool is not well formed . It is semi solid and not cylindrical. Earlier my stool was cylindrical with cracks on it. The stool is semi-solid even after i stopped consuming milk and other foods causing irritation. I consulted a doctor and he did stool test and blood test. Nothing was found in that test. A colonoscopy , endoscopy and biopsy was perfomed. during endoscopy H.pylori was detected and combination antibiotics were given for 10 days. According to biopsy report, doctor is now reccomending CT scan of abdomen . He suspects tuberculosis/Chrons disease. I have stopped taking food from outside and i am cooking for myself. what is your thought and advice on my condition?

Detailed Answer:
Hello
Welcome to HCM
Thank you for your query.
I understand your concern
Well worthwhile getting an second opinion on the biopsy of the colon (terminal ileum).
Also you have not mailed your colonoscopy report too.
In addition try getting an test called fecal calprotectin and stool for giardiasis tested on three different days.
Then can consider getting an CT scan or an capsule endoscopy to look for a abnormalities in small intestine.
In addition try taking a course of Nizonide for 3days after consulting your physician.
Also adding a probiotic like vsl 3 once a day will help you.
Hope this answers your questions.
Happy to help.
Wish you a speedy recovery.

Hi,
I have uploaded the colonoscopy report.I have also uploaded the first stool sample test result.I have checked with my doctor regarding giardiasis. He told me that ,since I have taken combination antibiotics(Metronidazole, amoxicilin, clarimthromycin) for killing H.Pylori for 7 days , parasite giardia will be dead. However i have given my stool sample for testing. I have to give two more samples to complete stool testing 3 times. After taking those antibiotics I don't feel any trouble. Digestion seems normal now a days. I will wait for 10 days . If there are any problems I feel during these 10 days , I will reply back.
I tried to get a calprotectin test but its not available in our town.
my doctor has ruled out tuberculosis after doing a mantoux test. he told that for intermittent diarrhea, we can suspect crohns disease .

Detailed Answer:
Sorry for the delay in replying, I have gone through the attached colonoscopy report, and everything seems to be normal. Therefore, you do not need to do a fecal calprotectin test as this is a new form of test used to look for inflammation in the bowel. Complete the course of antibiotics given for H. Pylori and consume clean hygiene food. Hope this helps,, Please feel free to discuss further.
Regards
