Brief Answer:
Needs work up and management accordingly

Detailed Answer:
Thank you for asking

I read your question and i understand your concern. With so much illnesses in your past history and heart problems, splenomegaly and chronic diarrhea may be connected or separate entity. Weight fluctuation should be evaluated for any infection or hematological pathology.

With hyperlipidemia, and that much medications, diarrhea can be a side effect of the medications. Hyperlipidemia in history on its own and use of lipid lowering agents can cause diarrhea.

Satiety with a few bites is due to splenomegaly and this splenomegaly needs clinical correlation with the causes, hematological causes like god forbid lymphoma or infectious causes like abdominal tuberculosis etc . Talk to yoru doctor and let them sort it out for you.

Apart from it , inflammatory bowel diseases and other intolerance diseases like gluten or lactose should be evaluated.

Meanwhile take BRAT diet, banana rice applesauce and toast, green tea and pomegranate juices.

I hope it helps. take good care of yourself and dont forget to close the discussion please.

Regards
Khan
