Question: What would be your first thought on a patient 32 yrs old, mother of 4 children, experiencing chronic itchy ears, (more than 2 years), recurring strep infections, recurring UTI's, all over body discomfort & pain, itchy skin, ear infections, sinus infections, constipation, very low back pain during bowel movements, chronic fatigue, mood swings, tonsil stones, gall stones resulting in gallbladder removal, high bp....

Well, you seem to be having a lot of problems which have set in at a young age. I ll try to sort it out for you here.

1. When you are getting recurrent infections, it is important to rule out any immunodefiency syndromes which can occur at a later stage. For that we will have to check your IgG, IgA and IgM levels.

2. For so many multiple problems like chronic fatigue, mood swings hypertension we need to look into your autoimmune work up as well. As they can present with a lot of vague symptoms and may be hidden underneath

3. Get your thyroid and Vitamin D levels checked as they can cause fatigue and body ache

4. Lastly once we have ruled it all out, the last diagnosis of exclusion would be Fibromyalgia.

I would suggest if possible, we should do this as the minimum work up.

Results from CBC are within normal limits throughout. Interstitial cystitis possibilities will be further analyzed in one week.

Well I still feel you should get these tests done. If it a all normal then we are at least good that there is nothing serious. But if anything comes up we can take it up from there. Do let me know for more queries
