Prima Facie, based on the facts, it seems to be from the anal trauma from the applicator of the hemorrhoid cream. This rectal discharge also could be , just a liquefied cream,which you are using.
With the occasional discharge from rectum above causes need to be ruled out.
Other causes need to be investigated by physical and proctoscopic check up-with your doctor/Surgeon.

On reviewing the whole data,There seems to be Chronic infective lesion causing such a discharge.
Possibility of internal Fistula with draining abscess, Rectal Polyp with infection/ Cancer Ano-Rectal area needs to be kept in mind during investigating such cases like you.This is because, at this advanced age of 67,Cancer possibility needs to be ruled out by proctoscopy /Colonoscopy examination.
