No diabetes
I had full back surgery 10 years ago
There are 4 bones that are not adhered from the surgery. They are assuming that is where the infection is coming from.

Brief Answer:
It can be tuberculosis
Detailed Answer:
Hi,
There are very few infections which are difficult to diagnose like TB, fungal or Brucella. It is really frustrating because most of the symptoms like back pain, UTI ,weight loss, fever are non specific. Biopsies and culture is also negative.
Only solution in this is that clinician should start treatment by medicines and see if these are working.
My practical experience is that most of the time it is tubercular infection which cause such type of osteomyellitis. I would suggest you to discuss this with your doctor and if he is willing start anti tubercular treatment and repeat MRI after 3 months to see if there is any improvement. If yes then you should take full course.
I hope this answer will help you. Let me know if you want to ask any other question.
Thanks.
