What Causes Constant Chest Pain?

Question: Hello Doctor. For a few days now I have been feeling constant pain in my heart. The pain gets worse when I exercise. It also sometimes feels heavy to breathe. This seems to have started after I was drinking alcohol a few days ago. Also I should mention that I am under a lot of stress.

I have long qt syndrome and for that I take atenolol. Besides that I’m not smoking nor overweight. Blood pressure is slightly below normal and puls is normal.

What could be the cause of the pain?

Hi and thank you for asking!

You are reporting a feel of constant chest pain, worsened by exercise accompanied with dyspnea (heavy to breathe). The first things to be excluded in your case are cardiac problems. Moreover you have long QT syndrome which may be complicated with life threatening arrhythmias.

Once that cardiac problems are excluded than gastro intestinal problems (Gastro Esophageal Reflux Disease, gastritis and liver damage) are the most common causes of pain after heavy drinking.

If you were my patient I would recommend doing the following examinations:
- ECG
- Cardiac ECHO
- Stress test
- Holter monitoring
This will help in excluding any cardiac problems.

Only after doing so I would recommend doing an abdominal ultrasound and an upper endoscopy to rule out, or in any gastrointestinal problems. In case that all the examinations will be normal, than stress is the most likely cause of your concerns.
