What Causes Constant Fever In A Child?

Question: Hello Doctor,
We are currently out of XXXXXXX My daughter (5 years old) is having fever since Sunday evening. We used paracetamol for first two days then Ibugesic plus for next day. We also used Meftal syrup twice. After giving medicine the fever subsides. However, although at an irregular interval it keeps coming back. Today, early morning it went to 102 F. Her appetite has reduced but not a lot. In the first two days of fever, she was not very active, however, now once fever subsides she is active/ normal again. Could you please advise?

I have gone through the details and do understand your concern.
Most common cause of fever in this age is some viral infection. Fever due to viral infection usually shows a declining trend after 3 days, so monitoring of fever with temperature chart is very important.
Here I won't get worried as child is otherwise active when temperature is normal.
I suggest you to make a chart for monitoring of temperature.Continue giving antipyretic like paracetamol/ibugeaic or Mefenemic acid) with plenty of fluids.
Assuming the current illness to be a viral infection I would like to observe the child for one more day and if the severity or frequency of fever remains same, better consult a pediatrician for clinical evaluation and/or some investigations like complete blood count, C reactive protein(to look for any bacterial infection) and urine test.

Thank you Doctor. We consulted a local doctor. He recommended an antibiotic (amoxiciline Sandoz Forte 250mg/5 ml) as he observed redness in ear + throat ache + feeling of vomiting. Dose is 6 ml/ three time a day for 5 days.
We started dose yesterday afternoon. However, the fever kept coming. Last evening it reached 101.4 F. Had to give Meftal. After that in the night at 2:30 am, there was 99.5 fever so we gave Paracitamol. Since morning, there is no high fever however, fever has not completely subsided. even now it's 99.4 F. Daughter is active but not 100% active. I hope this is normal and fever will eventually go completely. She keep complaining about vomiting sensation. We are giving her Emeset. I hope that's OK.

As you have mentioned it appears that fever is already in declining trend. Mild lethargy and decreased appetite is normal during any febrile illness.
Amoxicillin works well in Upper respiratory tract and ear infection. The prescribed dose of amoxicillin is also right as per the age.
I won't get worried for complaint of nausea if there is no significant vomiting.
I suggest you to continue the same and hope that fever will subside within a day or two with this treatment.
