What Causes Constant Fever In A Child?
Question: My 8 year old daughter is having slight temperature from past 23 days or so. from 5th april to 15th april it was low grade, below 100.5 appetite normal no symptoms. from 16-21 it went to as high as 102.5. all basic tests like cbc, mentaux, culture blood and urine etc were done but all reports normal.
after high fever no fever for 3 days before again it went to 101 on 24 th april. second line of tests like malaria,dengue, ANA all negative. elevated ESR(59) and platelets 5 lakh in second round of haemogram. otherwise all normal. nowadays sometimes 98 in the morning, higher 100.5 in the evening. chest xray and ultra sound abdomen normal.
Current medications:
Doctor gave her broad spectrum antibiotics for a week when temp was 102.5

Hi..... I have gone through the details of your kid's case. But has the fever is still there I suggest two possibilities.
1. Prolonged viral illness.
2. Lymphoreticular malignancy.
3. Kawasaki disease.
4. Infective endocarditis.
I suggest that you get a bone marrow aspiration done for your kid and send the specimen for histopathological examination and also flow cytometry.
I also suggest getting a EBV VCA IgM done to rule out EBV infection. Get an Echo done to rule out infective endocarditis and Kawasaki disease.
