What Causes Constant Sharp Lower Right Abdominal Pain Around The Area Of Appendix?

Question: I have had constant sharp lower right quadrant pain around the area of the apendix and the bladder for the past four years
I am a 28 year old female and I have no appendix
Nothing was found during laporoscopy surgery, nothing found during TV ultrasound, Proctitis was found during colonoscopy but I was told that is not the cause
Pain is made worse by lifting my right leg, sitting up from laying down and pressing on the area deeply and firmly, if i take a deep breath and my tummy extends with air it agravates the pain and if i press on the left side of my stomach it hurts the area on my right side
My dr now thinks it's in my head
It feels like something is tearing inside me 24/7 it is sharp and knife like and is if im pressing on a bruise inside my body not one pain killer I have had for it including oramorph has taken the pain away could you help me?

First of all please note that it is very hard to help you from the point of this service. I am unable to examine you or look through all your reports.

Your pain seems to be caused either by muscles issue or something which is located close to the muscle (iliopsoas muscle to be specific). When you lift the leg the muscle is stimulated and gives pain.

As the pain also appears when pressing on the left side, intestine issue is possible (when you press on the left side, the gas moves back to the right part of the large intestine and can either push small intestine or the muscle).

If it were intestinal issue, you should also have some diarrhea and/or constipation, occasional bloating, cramps. During the laparoscopy, we are unable to check whole small intestine.
There might be also an adhesion caused by appendix removal surgery.

This are just thoughts. Like I have mentioned before, its really hard to help you, especially that so many tests have been already done.

But in my opinion you should have some more tests done:
- spine and pelvis MRI (to check for nerve and muscles issues)
- if you have mentioned intestinal symptoms, enteroclysis for small bowel diagnostic

I suffer with random bouts of diarrhea which usually happen without warning once every so many months
I usually go to the toilet once a week and so tend to be constipated quite regulary and suffer occasionally from cramps and nasuea
I have had a colonoscopy three years ago which showed proctitis
I also do not pass wind very often and feel very bloated and sluggish
I am now just recently losing my appetite and becoming full very quickly when eating
I have had a spinal mri about three years ago which was normal

CT enteroclysis is necessary.

Detailed Answer:
If you also have joints pains or some strange skin changes (like skin allergies), Crohn disease can be your problem.

Bouts of diarrhea along with bloating make small intestine issue much more possible. Usually partial bowels obstruction (due to some narrowing of the small intestine) act like that.

This makes CT enteroclysis (special test for small intestine) more than advisable.

Yes i suffer badly with joint pain and muscle pain and fatigued
And yes i have over the past two years developed on my legs patches of small red flaky skin from the thigh to the ankle that i have never suffered from before and i cannot shave because of them

Well, this makes Crohns even more possible. Please consult gastroenterologist. You may start with another colonoscopy (most of the lesions do appear in the small-large intestine connection - in lower right abdomen). If colonoscopy will be clear, go for enteroclysis.

This skin lesions are not characteristic for Crohns.

Detailed Answer:
Such skin changes does not look characteristic for Crohns. But it does not really rule it out. In general, Crohns is due to autoimmune issues. Same autoimmune reaction can be present in the skin and joints. Some can look like yours, but again, those are note characteristic.
