Your wife is having muscular pain in scapular region. Treatment of it is rest and analgesic such as ibuprofen for pain relief. If ibuprofen is not effective then strong analgesic such as tramadol can be helpful. muscular relaxant such as Thiocholchoside
is helpful in reducing spasm. Vitamin B and C is helpful in recovery. Icing is helpful. I found in my clinical practice good results with injection triamcilone locally. You can discuss with your treating Doctor about it.You can further investigate it by MRI of the region.

Pain should reduce within a week of starting treatment , but rest is very important to reduce it. It is advisable to rest and exercises are not beneficial. It is unlikely to be pneumonia as she has not having other symptoms like fever and shortness of breath . However she can get her x chest to rule out it. Rest already been discussed.You can discuss with your treating Doctor about it.
