Based on your description, the most likely cause for the pain is Gastritis. However, even with abdominal/epigastric pain a cardiac cause should be ruled out as it can present as such. The better option would be to give your wife some over the counter antacids like peptobismol or H 2 blocker like ranitidine or even better a PPI like omeprazole and get her to an ER near you. An EKG can be done. If the EKG is clear and the pain has persisted despite the OTC remedies then IV PPI infusion along with antispasmodic can be advised in consultation with your doctor. If these episodes recur despite treatment with PPIs then an upper GI endoscopy and H. Pylori workup should be considered as the next step.
