Thanks for writing to us. Followings are my comments:
1) Vaginal bleeding after childbirth by C-section usually settles down within 2-4 weeks in most cases. All the changes, occurs during pregnancy, return back to non-pregnant state 6 weeks following childbirth.
2) Reasons for persistent bleeding beyond 6 weeks could be retained placental tissue or amniotic sac membrane in the uterus (though rare after C-section).
Other cause is infection of the lining of the uterus or endometritis. Blood test for WBC count and ESR should be done along with culture test to rule out causative organism.
It needs proper treatment with a course of Oral or IV antibiotic (1st or 2nd gen Cephalosporin). Doxycycline and Metronidazole may be added.
3) If it still persists, I would suggest her to get examined by her gynecologist to look for cervical or vaginal causes for bleeding. If pelvic examination is normal, one Trans Vaginal ultrasound scan (TVS) may be helpful to identify uterine or cervical causes like polyps, fibroid or endometriosis.
4) Suspected choriocarcinoma (very rare condition) should be ruled out. It can develop following delivery and involves some of the placental cell, continuing to grow, despite the pregnancy being over.
It can be a very dangerous condition, but never gets better on its own. Blood test for beta-hCG along with TVS scan is usually advised. Chest X-ray should be done after that.
5) In the event that ultrasound scan and blood tests are normal, it is more likely to be hormone related cause. In that case, cyclical hormone therapy (Progesterone) may help to control bleeding.
Lastly, I suggest her to consult with OBGYN for above mentioned investigations and treatment with complete prescription. Maintain genital hygiene properly.
