I will analyze all your points and provide necessary explanation and treatment.
1. You have wound over right index or point finger since few months!
Yes.. I am sorry to say this but in the setting of Diabetes, it definitely takes a more time than normal expected time.
Do not worry, it should heal..
2. Bleeding and you say nail gone.
If it's nail bed infection, then yes. It takes time to heal.
3. Epsom salt and hydrogen peroxide
Yes they are good. But I suggest you not to use peroxide unless there is slough in the wound.
Slough meaning, whitish tissue
Look, generally, the wound should get red or pink.
I saw your wound, looks healthy and red.
Is there some kind of smell or discharge apart from blood?
If no, then I suggest
A. Surgically debride the tissue under local anaesthesia.
Local anaesthesia is enough since you are old aged and only finger is involved.
B. This will be followed up with skin grafting.
Small piece of skin taken from other parts, generally thigh.
This is the treatment of choice according to me, on your case because you are fed up with the bleeding and the wound.
C. Good sugar control is a must.
Most important thing is control of your diabetes..
4. Yes with your medical history of defibrillator its better that you undergo this procedure under local anaesthesia, it's safe..
