The common cause of cough and cold in a 4 month old baby is viral infection(bronchiolitis). In this condition the baby may have increased respiratory rate with retraction of chest and noisy breathing due to spasm of airways.
The main aim of management is to supplement O2 to prevent hypoxia,to decrease spasm and to maintain adequate hydration.
The nebulisation helps in rapid relieve of spasm of airways.
Both the ascoril and deletus bx syrup contains terbutaline(bronchodilator,to relieve spasm and cough) and ambroxol(mucolytic to make the mucus thin).
The terbutaline strength is 1.25 mg/5 ml in both the syrups.The dose of terbutaline is 0.1-0.2 mg/kg/day.
The average weight of a baby at this age is about 5 kg.So the dose will be around 1 ml three times a day.
Although it is a matter of debate that which bronchodilator we should use but we have seen that terbutaline helps in relieving spasm and thus respiratory distress in this age.
You can give one of these syrup safely.Also give nasal saline drop to relieve nasal blockade and paracetamol if there is fever.
Try to feed her frequently.

The tremor is one of the common side effect of terbutaline.
The other side effects of terbutaline is very less and it can cause sweating,palpitation or irritability.
I won't be overly worried in the present situation and would like to decrease the dose if the condition persists.

The bowel pattern can vary.This variation is not due to the medication.
It might be due to dal ka pani that you have started recently.
The baby's bowel pattern can change after introduction of any new cereals/ pulses or other food material.
I won't be worried by this change in pattern of bowel unless it is too watery or too frequent.
The increased frequency may also be due to the current illness(may be viral).
So I would like to continue the feed and medication as usual and suggest you to observe the baby for any increase in frequency,associated vomiting,decreased oral intake,excessive lethargy or irritability.
If you notice any of these consult your pediatrician.
