Question: I have been having daily RUQ discomfort/pain, daily loose yellow/brown bowel movements, occasional nausea, no weight loss. Had gall bladder removed 20 years ago due to gallstones. No matter what I eat or how much I will have urgent loose bm's within 15-30 mins after eating almost every day. Gave up dairy without improvement. Stools are often "oily" Ideas?

Detailed Answer:
Hello XXXX and thank you for asking HCM.

I have read your symtoms and together we will find a solution.
Your symptoms are:
-right upper quadrant discomfort/pain
-daily loose yellow/brown bowel movement
-occasional nausea
-stools are often "oily"
-problems with your stomach like zollinger's disease etc.
You had surgery 20 years ago due to gallstones.

I will start from the bottom of your symptoms. Having oily stools (steatorrhea) is a symptom for some problems like :

-acute/chronic pancreatitis
-common biliary duct problems(stones, cholangitis etc.)
-pancreatic enzyme deficiency (exocrine insufficiency)
-absorption problems (IBS-Inflammatory bowel disease...)

Now there are times when in the stools there is mucus, so to make a differential diagnosis you need to do a stool test (routine microscopy). If there is fat in your stools then other tests needs to be done for the diagnosis.

If I're your caring doctor I would recommend these tests :

-ultrasound of your abdomen to check the pancreas and the common hepatic duct
-blood tests, especially liver enzymes and pancreatic enzymes
-stool test

After the results of these exams, we will be able to tell what is the problem and how to treat it.

If you have any other question, feel free to contact me.
Wishing you good health.
