What Causes Dark And Dry Skin When Undergoing Depigmentation Process?
Question: Hi doc...
M XXXXXXX ... i have been going through depigmentation process from past 2 months... including melacare plus... peeling... lately my doc gave me a cream dermz lite which had hydroquinone 2%.. tretinoin.0.025%and mometasone furoate .1%..it peeled of my skin on chin and my skin became severely red ... there was lot of inflammation ... my doc gave me dosetil to reduce inflammatin... next day wen i woke up i saw that red skin on my chin had become dark brown and dry... my doc is saying that m getting worried without any reason... but m really worried ... i hope the dark brown dry skin will chip out...

Thank you for writing to us on HCM.
It seems you have got contact dermatitis to Dermzlite cream and eventually post inflammatory hyperpigmentation.
As of now, omit all topical creams to prevent further damage and just apply a good moisturiser like Cetaphil DAM thrice a day for rapid healing.
For better management, I insist you to post good clinical pictures to me so that I can guide you properly.

This doesnt seem to b pigmentation... it is like a dead dry skin has covered that area... if i cud i wud hav send u the photograph...but my fone camera quality is not that good u will really not understand.

Detailed Answer:
Okay no problem.
The dead dry skin you have referred to must be crusting formed due to excessive irritation by use of Dermzlite cream. Such crusting also forms after chemical peels.
Once this crusting is peeled off, newer and rejuvenated skin beneath it is visible. But this rejuvenated skin is highly sensitive and can get pigmented due to various reasons especially from sunlight.
Thus I would advise you the following treatment protocol:
1. Avoid further application of any medications like Dermzlite.
2. Apply mometasone cream( Momate) twice a day over dead-dry patches of skin with gentle hands.
3. On the top of mometasone apply ample amount of moisturizer.
(Cetaphil Dam) to cover whole face.
4. Use good quality high SPF sunscreen everytime before going out in sun. (Photostable sunscreen or Sunscote Sunscreen gel)
5. Avoid pricking/picking/peeling of dead skin.
6. Use gentle cleanser like Cetaphil Cleansing lotion to rinse your your face.
I am sure, with the above treatment, your condition improve pretty soon.
Please do report me your condition after 10 days.
