What Causes Decreased Urine Output And Painful Urination?

Question: Doctor, In XXXXXXX 2015 I had a problem with my flow and consulted urologist and taken all tests including blood test for PSA and abdoment ultrasound and all came OK. The doctror checked my urthera and said very very small block at the neck but said it will go away with medicines. I took selodil i think for 3 weeks and OK. However from the last couple of weeks, I have burinng sensation after I pass urine and have buring sensation in anus , however my flow is similar and no change. I stared homeopathic treatment for my digestive issue for the last 3 weeks and checked with him the issue of burning sensation and he says these symptoms will go away and need not worry. Please advise

Thanks for your query ,based on the facts that you have posted it appears that you have poor flow of urine and urologist who on endoscopic examination detected bladder neck obstruction and prescribed Selodil once daily .

I shall be happy if you can post me few more details to help me in making comments on your problem.
1) Is it possible for you to post ultrasound Scanning report ?
2) Have you done uroflometry ? and what is maximum flow rate ?
3) Do you have painful ejaculation now ?
4) Did you pass few drop of blood with stools ?

Your present problem of burning urination and burning in anus could be due to Acute fissure in anus or Prostatitis .

You need to consult qualified General Surgeon for digital rectal examination .

In the mean while take antibiotics like Ciproflaxacin twice daily along with topical local anesthetic ointment like Nupercainal for local application in anus .

Selodil acts as a smooth muscle relaxant in bladder neck and improves urine flow hence has to be continued for long time ,discontinuing Selodil will again lead to poor urine flow .

Thank you Doctor for the answer . Regarding

1. Ultrasound scanning report. I could not scan it but the final remark says
"NON REMARKABLE IMPRESSION"
2. Uroflowmetry I did it is lower than the lowest percentile
3. There is no major pain while ejaculation. Little pricking kind of feeling I get
4. No drop of blood in the stool

I want to give you some more update. When I stand and walk I don't feel any pain / burning sensation and similarly when I sit at the edge of the seat or sofa I don't feel any pain / burning sensation. However, when I sit in the normal posture and when I am lying down I get the pain/burning sensation both in my anus and penis.

Is there any serious problem should I need to worry about ?

I will continue Silodosil and consult General surgeon. However would the local factor / local issue would cause pain / burning sensation in the penis and rectum and groin as well at times ?

Any pathology in anus causes discomfort in genital area .This is called as referred pain due to fact that all these organs are having same nerve (Pudendal Nerve) innervation .

Thank you for the quick response Doctor . Hence the local factor can cause 24 X 7 pain the anus and discomfort in genital area ? Except when I walk , all other times when I sit or lying down I have the pain .

Pressure or irritation over anal region may cause referred pain .

It is difficult to explain the precise cause unless local examination in person is carried out .

General surgeon after examination will be in a better position to explain the cause of your symptoms .
