If I would get to treat a case of OCD like you, my approach would be to add an antipsychotic drug like risperidone to SSRI (from among fluoxetine, paroxetine or fluvoxamine).
If there is no response to this regime after one month, I would add mirtazapine to SSRI.
OCD is a difficult to treat condition but an appropriate drug combination can help in many cases.
Along with drugs, psychological treatments like exposure and response prevention are also helpful and must be started under supervision.
Your mental confusion at night may be due to latuda.

Both latuda and klonopin can cause confusion.
Risperdal did not work for you.
You have these thoughts 24 hours with urges.
It would qualify for OCD if you know and understand that these thoughts are irrational and persist against your will.
You were too sedated with risperdal. In that case, low dose of solian (amisulpride) or abilify (aripiprazole) should help you (in combination with SSRI). These are also antipsychotics but will not sedate you.
(prescription drugs).

The person may think about a time when some bad/harmful event might have occurred and have a “memory” about that event.
