What Causes Diarrhea While On Dilaudid?

I did have a run in with c diff 2 yrs ago but that was successfully treated. I am on the blood thinner xarelto 20mg once daily due to blood clots behind my knee. I will be on it until may for sure but possibly for life.
I notice the episodes are worse when I've eaten dairy, food or drink containing splenda artificial sweetener or wheat.

Hi,
I have gone through your medical history carefully and understood your concerns.
1. Your current problems with loose watery stool with various amounts of blood need further evaluations:
- have you been suffering of constipation after or before the surgery?
- have you changed your diet after surgery?
- any personal history of hemorrhoids?
- have you been using antibiotics recently?
2. If the watery stool are worse when you eat dairy food or drink containing splenda are suggestive of food intolerance. However, infective causes should be ruled out first (e.g. a relapse of c.diffic? or candida infection due to alteration of intestinal flora after antibiotic use).
3. Stool culture and examination for ova/parasites is suggested together with physical examination of anal area to check for possible hemorrhoids or fissures). Colonoscopy is also advised to check rectal mucosa.
Looking forward to have the requested information in.

Yes I had horrid constipation after surgery and went almost a week without a bowl movement until the senakot the pharmacist suggested cleared that up quickly and violently
Other then eating more then usual amounts of dairy, wheat and splenda I have not changed my diet
Yes I was diagnosed with internal hemorrhoids 5 months after the c diff infection during a colonoscopy. I had that because I was still suffering symptoms even though the infection was cleared with metranizadole
Yes I was on antibiotics 2 weeks before surgery to clear up bronchitis....I can't remember the name but it was take 2 pills the first day then one pill per day for another 9 days totaling 10 days treatment. As well I can't remember if the orthopedic surgeon gave me iv antibiotics during surgery but I had none by mouth or iv afterwards
C diffic/candida infection to rule out through culture of stool...
Detailed Answer:
Hi back my dear,
Thank you for providing the requested information.
From the additional data you are providing, all your current health problems are due to constipation:
- antibiotics you used have aggravated constipation and at the same time altered the intestinal flora. To my opinion, you might have another bout of c diffic infection; stool culture should confirm it.
- the presence of blood in the stool is due to hemorrhoids lacerated while passing very hard stool with difficulty
For the moment, I advise to:
- increase fluids intake to prevent dehydration (water with OTC ORS)
- take probiotics to stabilize intestinal flora
- take tasectan or pepto bismol to stop diarrhea
- if you feel any lump in the anal area (possibly hemorrhoids); should keep good hygiene of the anal area; and apply anti-hemorrhoid cream locally
Once again, you need to run culture of stool to exclude c diffic and candida infection; if present, should treat them adequately.
All the best!
