Question: GOOD DAY SIR,
I am looking for best treatment for treating on of our relative aged 85years female. she is suffering from ILD her health is detoriating day by day and she is feeling difficult to even to take food. we will attach medical reports based on your advise.

Hello dear, thanks for your question on HCM.
I can understand your situation and problem.
ILD is having many types. By your history and description I think she is having IPF (idiopathic pulmonary fibrosis).
It is having worst prognosis amongst all ILD.
It is chronic, progressive and end stage lung disease.
Till date no sure treatment is available.
The best treatment is oxygen supplements.
Steroids are useful in exacerbation.
In my opinion, she is having exacerbation of IPF, at present.
Please let me know her CT thorax report, PFT ( pulmonary function test ) and chest x ray (if done recently).

Dear Sir,
While thanking you for your prompt reply. We do not have CT THORAX REPORT AND PFT reports. We will try to get soft copy of chest report. I will send the reports ones ready. Please advise us we can use any incubators and apply oxygen periodically.

Please let me know
1. Does she have fever?
2. Does she have expectoration?
3. Does she have edema feet?
4. Have you checked for oxygen level?
I am asking all these to rule out infection because infection is the most common cause for worsening of ILD.
Better to give her continuous oxygen at 1-2 lit/min.
Please reply me answers of above asked questions, so that I can guide you better about antibiotics.

Dear Sir,
She does not have fever, and also have no expectoration,her feet is slightly swollen. Her oxygen level is not checked. Any additional information will be forwarded to you.

Guve her low flow oxygen at 1-2 lit / min.
And better to give her broad spectrum antibiotic.
Does she have acidity problem?
Is she taking any drugs, like steroids or bronchodilators etc?
