Brief Answer:
Could be due to Urethral stenosis

Detailed Answer:
hello
Thanks for query .
Inability to pass urine or having to strain to pass urine could be due to one of the following factors
1) Urethral Stenosis (narrowing of urethra) which is common in perimenopausal age group
2) Impacted stone in urethra
3) Spasm of the urethral sphincter due to UTI .

Consult qualified Urologist for clinical examination and get following basic tests done to confirm the diagnosis.
1) Urine routine and culture.
2) Ultrasound scanning of abdomen and pelvis
3) Endoscopic Examination of Bladder (Cystoscopy) with calibration of urethra (if required )

In the mean while you may start taking antibiotics like Cefixime and Nitrofurantoin twice daily along with urine alkalizer Citralka thrice daily .

Ensure to drink more water

Further treatment will depend upon result of these tests and final diagnosis.

Hope I have answered your query ,please feel free to ask if you have more questions,I shall be happy to help you
Thanks and Regards.
