What Causes Discoloured Urine Along With Foul Smell?

Question: I am a retired physician. TURP about 15 years ago. Because of an atonic bladder wall, I have to catheterize myself q 7-8 hours. I am on Coumadin. For the past 4 days, my urine has been mahogany-colored and notice a fishy-smell. Please advise. Thank you.

I have gone through your query and would try to help you in the best possible way.

My advise:
I understand your concern and will put some possibilities in front of you and some suggestions to consider.
POSSIBILITIES:
1- Infection: Sir! being a physician yourself, you must be of the knowledge that there is a high chance of urine infection in patients who undergo frequent urinary catheterizations.
2- A Stone or erosion bleeding:
There can be some erosion independent or due to a stone which might be causing this mahogany-coloured urine
3- Bleed due to Coumadin:
Some times patients on Coumadin may bleed in their urinary tract with sometimes no evident focus of reason of bleed.

SUGGESTIONS:

URINE ANALYSIS:
Sir! all the three points i mentioned above can be ruled in, or ruled out, by getting a complete urine analysis done , you will either get casts which which prove it's tubular bleed or XXXXXXX blood which will prove it to be bladder or urethral bleed or you will find WBC to rule it in, as UTI.

Then if it's a bleed your doctor can work out, the cause of bleed and if' it's an infection you can get a culture done and can start medication.Either way a urine test will help you and cover up all the three possibilities I have put in front of you.
