Brief Answer:
Oxygen therapy will be the cornerstone of management.

Detailed Answer:
Hi,
Thanks for posting the query on HCM. After going through your query, I would like to comment the following:

1. Your husband seems to be suffering from IPD (Interstitial Pulmonary Disease) commonly known as ILD (Interstitial Lung Disease). the most common type is IPF (Idiopathic Pulmonary Fibrosis) in absence of a known cause.

2. His baseline oxygen saturation is below 90. With Oxygen if his saturation is more than 90% then that should be acceptable.

3. He does have additional cardiac comorbidity with implanted pacemaker which requires a regular medical followup with your doctor.

4. His complaints of abdominal discomfort across the waist line should be reported to your doctor. The discomfort could be due to musculoskeletal cause or internal causes. You need to report it to your physician.

5. His cardiac functions need to be monitored, especially his right heart pressures on 2d echo.

6. Does he have any leg swelling? Was he a smoker? Any other disease diagnosed?

7. A decline of lung functions to 60 % is expected in ILD especially considering your husband's age. You need to give him continuous ling term oxygen therapy. Also give him pneumococcal and flu vaccines in consultation with your doctor.

8. Its a hard fact but ILD is a progressive disease. The aim of the medical management is to slow the rate of progression of the disease as much as possible.

I will be glad to answer follow up queries if any. Please accept my answer if you have no follow up queries.
