Brief Answer:
Probably dehydration and reflux. No harm in checking it out with tests.

Detailed Answer:
Hi,

Thank you for your query.

1. This sounds like dehydration. Dryness in the airway leading to a tendency to hyperventilate and cramps.

2. Reflux will also worsen this feeling, sometimes after a gym workout. You need to review your diet.

3. The interesting thing about such conditions is that they are usually harmless, especially if this has been going on for a long time.

4. Since this has to be differentiated from a cardio-respiratory problem. I will recommend that you get two ECGs (Electrocardiogram's) done 6 to 8 hours apart.
A Cardiologist's reference can the be done. It will help if you get a Chest X-ray, Blood sugar levels, Blood Urea Levels, Sodium, Potassium, Calcium, Vit D, Vit B12, Thyroid function tests and A 2D Echo done. You may also get a Holter (24 hour ambulatory monitoring) done including a visit to the gym.

5. When all these tests are normal, the issue of the reflux remains (the feeling to burp, but can't). A trial with anti-reflux medication and a bland diet will be required for that. Get your Neck-X-rays done.

6. These are a lot of tests but when done systematically, will help settle your mind that everything is normal. Upload the results here.

I hope that I have answered your query. If you have any more questions I will be available to answer them.

Regards.
