I am Dr. Shama and I will be answering your question.
The symptoms suggested by you in the form of dizziness on tilting the head are most commonly seen in problems of inner ear like benign positional vertigo or problems in the neck like cervical spondylosis.
Benign positional vertigo presents with dizziness on movement of the head with some symptoms of inner ear like ringing in ears. This occurs because of misalignment of the vestibular apparatus in ear.
Cervical spondylosis occurs because of age related sclerosis of the cervical bones and presents with dizziness and pain in the neck and backache.
I would like to treat this condition by antivertigo drugs like cinnarizine for symptomatic relief.
Later I would suggest examination by ENT doctor and orthopaedic doctor to rule out benign positional vertigo and cervical spondylosis respectively.

Routinely TSH level is sufficientfor screening and if it is altered then T3 and T4 estimation is required.
Consulting ENT doctor is required to rule out ENT causes of vertigo.
I advised for orthopedic consultation as you are having low back pain.
