TIA workup would have been appropriate if there was no relation of the food intake with the vomiting. A single episode of vomiting that is associated with food intake appears to have more association with a gastrointestinal cause. It is likely to be food allergy that lead to histamine release and subsequent gastritis that presented as the vomiting episode. If the dizziness and vomiting ceased after the one episode of vomiting and there was no diarrhea associated then H 2 blockers like ranitidine along with metoclopramide can be advised. If the vomiting persists or diarrhea also ensues then antibiotics would be required as part of the management. The blood pressure should be checked and isotonic fluids like ORS should be given to prevent dehydration and electrolyte imbalance. You may get specific ANTIBODIES for the foods you mentioned tested for in your serum to find any allergic response to these foods. DESENSITISATION THERAPY can be initiated if any allergic response is determined.
