Based on the history given, according to my opinion you are having Irritable bowel syndrome, this usually associated with diarrhea or constipation. As soon you eat anything it stimulates bowel to pass stool.
Usually, with a few basic changes in diet and life style activities, Irritable bowel disease will improve over time.
1 Avoid caffeine (in coffee, teas, and sodas).
2 Add more fiber to your diet with foods like fruits, vegetables, whole grains, and nuts.
3 Drink at least three to four glasses of water per day.
4 Don't smoke.
5 Learn to relax, either by getting more exercise or by reducing stress in your life.
6 Limit how much milk or cheese you eat.
7 Eat smaller meals more often instead of big meals.
8 Keep a record of the foods you eat so you can figure out which foods bring on
bouts of this disease
If it is not reduced by following above instructions you may need to meet local doctor for further evaluation.
Dizziness is may be because of postural hypotenstion,in this as soon you gets up from lying position you feel dizzy,for this you need to sit for few seconds before getting up from the bed.
If your are having dizziness through out the day , you have to meet your local doctor and get done your routine BP , sugar levels and other blood routine tests done

things like caffeine ,smoke ,500 ml milk are included in my daily routine .
dizziness is almost throughout the day but not continuously but it feels when i change lying down or seeing below down.
Hope is it not serious ?
if yes ? i will go to get checked up by local doctor ? if require ,physician should be proffered? or i should consult any other specialist ?

Your dizziness need further cardiac evaluation. You can meet local general physician. he will help you regarding this , he may ask you to get done ECG and other routine tests.

I have consulted with local physician and she told me go for blood test.
In blood test report everything seems fine except one test for TSH =9.1 .seems it if for thyroid ,
Please share you opinion on how serious this situation is ,i do not have any heredity for such symptoms .

According to your thyroid profile , your TSH is high, which indicates that you are having hypothyoidism. Dont Get panic.It is treatable by taking tablets.
You have to meet endocrinologist /GeneralL physician to adjust the dose of tablet to be taken according to your physical examination and lab reports.
Doctor may start on tablet thyroxine 50mg/75mg , you need to take these tablets for next 3 months and after that again you have to get done thyroid test to check the status, depending on those reports change in the dosage of drug can be done according your hormone level.
