Hello
Welcome to Health Care Magic.
My name is Dr Muhammad Ahmad & i will look into your problem.
I have gone through your query and would try to help you in the best possible way.
I am sorry for what happened i understand your concerns and will try to explain things to you.
It's a pleasure to know that you are a health professional yourself and well aware of your condition and treatment options.
Yes it Happened after dose was reduced to half , but it was a standard procedure, doctors expect that follicular growth will keep going and they will release eggs but this didn't happen and system collapsed.
This is rare but it happens,,actually when you take FSH and LH through external sources, due to negative feedback body stops making it's own ...this causes a sudden drop if external source's dose is reduced, you as a nurse must be knowing that hormonal therapy and it's response varies with the patients.
In some a 50% dose reduction doesn't cause a big change in E2 but in your case it caused exactly 50% drop it means your internal systems and follicular development which should have resulted in egg production didn't trigger and E2 dropped exactly according to dose reduction.
WHAT SHOULD YOU DO:
Wait for a month or so and you can start on with a new cycle, actually hormonal trials are always trial and error method with time you and your physician will get to know how you respond and at what dose you respond. Things are expected to get better.
PROTOCOL SUITABLE FOR YOU:
Don't change protocol as yet stay steady with one protocol for some time i understand your concern that you are worried about your increasing age, but don't panic till 40 you have good chances , just don't lose heart, stick to one plan but keep discussing with your doctors about other options and when they think you should switch to other protocol , making rapid switches to other protocols are not going to do any good for you.
WHAT CAN YOU FACE:
As I told you it's a trial and error thing, it does follow guidelines but patient to patient response varies, for first few times you might suffer some unwanted circumstances but keep yourself up and running ... keep yourself steady there are 60-70 % PCOS patients who deliver normal healthy babies with no real problems...
It just needs good management keen follow up and care, with time you will start knowing how your body behaves and what to be done, don't think of changing your protocol so immediately. Give your body and your doctors time. Yes you can always take second third opinions from any other fertility facility but it's always better to follow one plan with one team for some time before switching.
.Hope I have answered your query. If you have any further questions I will be happy to help if not then don't forget to close the discussion and rate it.
Wishing you good health !!
Don't panic and take planned steps.

Thank you doctor you have been very helpfull and understanding. Sometimes i think been a nurse makes it worse cause u know that little bit more!! Im definately going to stay with my clinic and i have a really good RE who is lovely. I think maybe if they had just reduced my menopur to 112.5 instead of 75 things might have worked out differently. Do u have any idea what dose they might try next time? Its the short protocol i was on which i added orgalutron too on day 4 to stop premature ovulation. Im just so worried i will never have a baby
Brief Answer:
stay calm and hope for best.

Hi,
Thanks for writing back,
Yes you sometimes feel that ignorance is a blessing, I as a doctor can relate to this but on the other hand we can use our medical knowledge for our advantage.
You must be knowing what mustakes patients make and what are mental and physical requirements to stay at a good chance, this can work in your advantage.
I would ask you to stay calm and dont panic you have 3 gold and 5 silver years till you turn 40 and you have a good chance in them. Don't think that you would never be a mother you have got quite a good time left, in this time just follow a good team and their instructions never add or substract a med by yourself unless doctor asks you to.
About dosing i would say that this has to be decided by your doctor now that they must have seen how you responded to dose reduction so they might reduce the dose cautiously , this is truly a subjective decision by a doctor according to protocol, his experience and patient's response.
You can always discuss it with your doctor and ask what can be done to avoid what happened the last time. How doses can be adjusted or what else can be done.
I would like you to give yourself good time to follow a protocol and hope for the best. Mental stress nevel has good effects on hormones and body so try to avoid it. I wish you best of luck and hope that things would turn out to be great in next cycle.
I hope i answered your question , if you have any other related query I am happy to help, otherwise close and rate the discussion please.Wish you best of luck :)
stay calm and hope for best.
