Brief Answer:
As explained.

Detailed Answer:
Hi.
Thanks for your query, albeit short and specific.
Noted the history of a 60 year male having lower back ache and pain in testicles feeling queasy.
Wants to know whether hernia type or lifting atwith or work...
The commonest cause of such a problem may be epididymo-orchitis secondary to urinary infection with or without prostatitis and so on..
These conditions can cause pain in low back as a reflex mechanism.
I would advise you the following:
Get urine: routine, microscopy, culture and sensitivity.
Tests of blood.
Ultrasonography of abdomen and scrotum with color doppler .
Clinical evaluation and examination by your Doctor and reference to Urologist for diagnosis and further management.
I hope this answer helps you.
