Question: Have dull pain in left side underribs area. Feeling sick, no appetite and no desire to smoke cigarettes which is rare for me i am 59 and obese 144 kg at 175 cm height

Hi XXXXXX ! Good evening. I am Dr Shareef, a general surgeon answering your query.

If I were your doctor, I would at the first place put my utmost efforts in dissuading you to stop smoking permanently, and any time you do it would be best for your health. Also you might have to think of avoiding alcoholic beverages if you do consume these.

Considering your smoking habit and age, after a general physical examination with special attention to your chest, I would then go for some basic investigations like a CBC, ESR, Blood Sugar, Blood Urea, serum creatinine, serum lipid profile, serum uric acid, serum amylase and lipase, a liver function test, a base line ECG, an X ray chest, and an ultrasound of the abdomen if need be. Further management would depend on the clinical findings of your abdomen, and the investigation reports.

Till then I would suggest you with a anti spasmodic, a proton pump inhibitor, and an anti emetic if feeling sick in that way.
