Well presence of fatty liver on ultrasound and normal liver enzymes suggest a diagnosis of fatty liver, a relatively benign condition with no obvious ongoing liver damage.
But this fatty liver may further lead to inflammation or swelling in liver ( termed as Nash) as seen by rise in Ast and alt levels with suggest an ongoing liver inflammation and damage which needs to be evaluated and confirmed first by ruling out other causes of liver disease like hepatitis b, c, alcoholic liver disease, XXXXXXX autoimmune liver disease etc.
Further after the diagnosis the initial treatment is lifestyle modifications like diet control weight control, regular walk etc if no response other treatment options like vitamin E etc can be added.

Also some times ultrasonography may not pick up fat, biopsy of liver is gold standard, but not necessary most of times
