What Causes Elevated PSA Levels While Treating Prostatitis?

default
Posted on Fri, 29 Apr 2016
Question: I had a PSA of 1.5 11/2011. Urosepsis 9/2012 with subsequent episodes of prostatitis requiring antibiotics until a long term antibiotic therapy was started last year. PSA 12/2015 was 4.2 and PSA 2/2016 was 4.5. Is the rise in PSA due to the long battle with prostatitis or more of a cancer warning.

Brief Answer:
Stop Bactrim at once. Send latest reports if available.

Detailed Answer:
Hello Kuri, welcome to HCM.
As an Urologist, I can fully understand your concern.

First of all, kindly stop the Bactrim at once.
Antibiotics are always as cyclical chemoprophylaxis, hence changed every 3 months.

I would like to know the latest available reports of the following :

When was last DRE(rectal examination) ,by Urologist done ?

1. Urine routine and culture.
2. blood routine, creatinine, RBS, and LFT, if done.
3. Ultrasound -KUB and TRUS .

After seeing these reports, I can advise you on further management.

A PSA of 4.5, isn't alarming.
Don't lose sleep over it.
It may become normal, after next test.

Regards
Dr.Matthew J. Mangat.

on 12/2015 creatinine 1.18, RBC 4.75, DRE was normal. Urinalysis of 12/2014 was normal. The symptomatic prostatitis episodes after 9/2012 occurred every 2-3 months and cleared with Bactrim DS BID x 4 weeks with each episode. Single strength Bactrim QD was started this January( after the last symptomatic episode cleared) and was ordered to be taken for one year. Recently the frequency of nocturnal urination has increased from every 3 hours to every 5-6 hours and I now have a sensation of complete emptying.

Brief Answer:
Tests done are not complete.

Detailed Answer:
Hi again.

You should get an urine culture.
I would like to know one more detail.
Is your RBS normal ?
An uroflow test is essential.
You should get an ultrasound scan-KUB, with residual urine, and a TRUS.

The Bactrim isn't indicated at present, if all reports are normal.

Hope I was heplful. Kindly get back to me if you have further query.
Wish you well.
Dr.Matthew J. Mangat.
