What Causes Elevated Bilirubin Level And Liver Enzyme?
Question: Hi,
My mother in law is 71 yrs of age and she got diagnosed with gallstones last night. Her other liver profile tests show billirubin: 5.88, SGOT:1291,SGPT:1768 .. Her Urin tests are normal but cholestrol is very low (102). She is diabetic and has hypertension. We will consult doctors on Monday however, after looking at her reports today, we wanted to take an expert's opinion on the treatment and risks(if any).. Please advise and ask more questions about the report as I have a detailed report of all blood and urine tests..

Going by the reports there is elevated Bilirubin level and Liver enzyme. In the presence of gall stones it is likely to be due to blockage of common bile duct( tube like structure transporting bile from gall bladder into intestine).
Low Cholesterol is good indeed. I would require additional information
1. What are her present symptoms?
2. Was Ultrasound or MRCP done?
3. Is she having fever?

Ultrasound was done last night and she was diagnosed with stones. No ulcer was detected. She had 99.8 fever yesterday but today she is fine. She has lost 10 Kgs' in the last six months and has symptoms of slight memory loss. Please tell me if we need to get worried about this, I mean is this serious.

Hi
If the ultrasound shows dilatation of Common Bile duct(CBD), which is very much likely in view of raised Bilirubin and presence of fever. She will require Endoscopic Retrograde Cholangiopancreatogram (ERCP) and stunting of duct to help in drainage( Raised Bilirubin level along with raised enzyme could be due to slipped stone from gall bladder blocking the CBD).
I would suggest you to consult General Surgeon/gastroenterologist for further assessment.
