I read your query very carefully and understood your concerns.
Your symptoms are typical for what we call morning hypertension. To treat it first it should properly evaluated.
If you were my patient I would recommend:
- Blood work
- Ecg
- Cardiac echo
- Holter and BP monitoring
These will help first to diagnose morning hypertension and than to differentiate between the two types of morning hypertension.
There are two types of morning hypertension hat need to be differentiated from each other
1. Morning surge hypertension and
2. Nocturnal hypertensive morning hypertension.
Can you show me your BP values in the morning and evening?
The treatment of morning hypertension is based on:
- healthy life style especially salt and alcohol restriction
- dividing ace-i (enalapril, ramipril etc.) in the evening and morning
- the use of diuretics
- the use of alpha blockers ( Prazosin Doxazosin etc.).
Can you upload the list of medications you are taking (or you have used in the past and didn't work)? These will help me to give you a more accurate treatment strategy.

Hi back,
After reviewing your treatment and other data my opinion is that you have morning surge hypertension.
If you were my patient I would have ordered lisinopril at bedtime and would add a diuretic (hydroclortiaside, clorthalidone, indapamide etc.) on your therapy (to be taken in the early morning). This combination will be more effective with less side effects. However you should discuss with your doctor about any change on your therapy.

The early morning hypertension surge is caused by the increased activity of sympathetic nervous system in the early morning. This is also accompanied with excessive release of adrenaline and cortisol from the surrenal gland.
As a combination of these mechanism you will experience a surge of hypertension in the early morning.
