Question: What can cause an enlarged uterus, bloating, and daily pelvic pain that is worsened by intercourse and/or strenuous activity at work?

Hello,
Thanks for the query to HCM for an opinion,
The most common cause of an enlarged uterus or bulky uterus as seen in ultrasound is ADENOMYOSIS which is a form of myometrial endometriosis. This can cause heavy and painful menses and is often accompanied by pain during intercourse and a constant pain in the lower abdomen which may be secondary to pelvic endometriosis and adhesion formation.
It is suggested that you should follow up with an MRI to look for any adhesions though diagnostic laparoscopy is the gold standard. Ideally if the family is complete and pregnancy is not desired infuser, then hysterectomy is the final treatment of the cause else medical amenorrhoea produced by drugs like progesterone only pills or GnRh analogues or implants or MIRENA is can help reduce the pain and further growth. Please discuss the options with your physician.
For pain relief NSAIDS and anti prostaglandins are the main source of symptomatic management in such cases,

I have had a transvaginal ultrasound.

Uterus L= 8.7x4.7 W=5.9
Right ovary L=2.4 W=2.5
Left ovary L=3.7 W=2.4
Endostripe=1.2mm

I've also had an endometrial biopsy that was normal.

NSAID pills have not given relief and make me feel sick to my stomach and cause palpitations. I'm currently waiting on a preop appointment for a hysterectomy.

Do you think I should ask for an MRI being that I am getting a hysterectomy soon?

Hello,
Thanks for the follow up.
I personally feel that it is always better to diagnose an issue first completely before we undertake any surgical intervention with the help of non invasive radiological imaging available at our discretion. It may also pick-up any adhesions in the pelvis, the knowledge of which would be of definite assistance e during the surgery as well.
If the route for hysterectomy is laparoscopy then it would be all the more advantageous.

I agree. I was not given a diagnosis but because I'm concerned with this daily pain and discomfort my ob/gyn suggested surgery because of it. He did tell me that there were numerous tests I could undergo but in the end I may still end up needing a hysterectomy which is why I decided to get one. I researched my symptoms and was thinking that I have what you said ( Adenomyosis). I was also a bit worried it could be the early stages of ovarian cancer because I have many of the symptoms.

I agree with your approach as well. hysterectomy in either case is warranted , so i think you should go ahead with it.
Your concern about ovarian cancer is unlikely to be true as the ovarian sizes are normal and there have been no adnexal masses found arising from the ovary on USG. Hence unlikely.
So you can go for for total laprascopic hysterectomy with bilateral salpingectomy but leaving both ovaries , considering your age and ovaries are functional.
