I am 27year old male 5'10'' and weigh 145. For the past two
years I have been experiencing extreme fatigue and hair loss.
I have done some lab work everything is normal except the below.
Please have a look
CK810
AST88
A trace of leukocytes in urine
detailed evaluation needed
Detailed Answer:
Hi, thanks for posting the concern in HCM.
Considering you history and information available from previous queries I understand that-
you have two current symptoms-
Hair loss
Extreme fatiguability
the following tests were found to be abnormal-
Vit B12
Creatinine Kinase
AST
As was outlined by our doctor in the previous thread, the problem seems to be the involvement of your muscles. Now, I want to go into a bit details.
The problem can be a muscle dystrophy, rhabdomyolysis, or a neuromuscular problem like motor neuron disease, particularly ALS (amyotrophic lateral sclerosis).
The latter seems to be more pertinent in the setting of B12 deficiency.
However, tests should be done to exclude renal problem as well, because same can happen in renal insufficiency.
Lastly, elevated CK, AST may also be due to cardiac problems.
So, a battery of tests are needed. From the history, I perceive that the routine investigations are normal. So, I would mention some special tests-
You may need Needle EMG, nerve conduction studies, MRI spine, Echocardiography and ultrasound examination of whole abdomen
For better evaluation you need to consult a neurologist as early as possible.
Please continue B12 supplement till further advice.
Hair loss may occur in any stressful condition to the body, including vitamin deficiency. A healthy lifestyle including a healthy diet and exercise and treatment of underlying cause would help you solve the problem.
