What Causes Extreme Paranoia, Knocking Sound In The Ear And Anxiety?
Question: My sister is 85, and she is very paranoid, thinking everyone is stealing her blind. She even comes up with these stories why they would do this etc. She also is hearing knocking every night, and also they have started coming in to her apt., and she has her bedroom door shut, and they say her name. I am her Power of Attorney for health. Her MD is not doing anything. Told her the other day she should see a psyche, and to take her anxiety medicine in morning instead of at night. She won't do this as it makes her sleepy. What should I do. I am her Power of Attorney. Thank you.

At her age, the most likely diagnosis here is dementia. Dementia can result in extreme paranoia. This is usually treated with a very low dose antipsychitic such as Seroquel or Zyprexa. She would benefit from taking one of these medications. I would mention this to your doctor to see what they think about prescribing one.

Wondering if she should have MRI to check brain for brain tumor, and testing for Parkinson, as I see that Parkinson can call being paranoid and have halucinations. I can check to see if she is already on these meds, and if not have the doctor prescribe them for her. Just wondering if should have a specialist in alzheimers/dementia see her and run some tests.

I completely agree with a specialist consultation to conduct a physical exam for Parkinson's and also for an MRI of the brain. However, a family history of Alzheimer's does make Alzheimer's more likely.
