Question: I was checking symptoms because of frequent faint, and sometimes strong, nausea, especially if I bend over (gardening is my hobby). I have a life time history of constipation. I am 72 with high blood pressure controlled by medication.

Brief Answer:
It could be vasovagal, neurogenic, cardiogenic or anxiety.

Detailed Answer:
Hi
Welcome to health care magic,

Frequent fainting while in standing or bending forward from working condition is known as syncope or a presyncope.

It means person suffers fainting spells without actual becoming unconscious for a while.

Here,I would like to ask you some questions regarding medical history (apart from hypertension),history of medications, any recent or past surgery, family history of heart problem, any past history of head injury ( like in accidents) or since how long you are having fainting spells or is there any other symptoms like chest pain, palpitations, fatigue, blurring vision etc.

We must rule out causes of syncope from cardiac and non cardiac for which you will require detailed history, thoroughly clinical examination for heart like ECG,blood pressure measurements with and without medications, echo cardiography etc.specially in persons who is above the age of 60 years.

The underlying cause could be vasovagal, orthostatic hypotension or postural tachycardia ,neurogenic syncope or syncope due to anxiety etc.

You will have to consult your physician first for thoroughly clinical assessment to detect the underlying cause.

Treatment will depend on the underlying cause.

I don't think your fainting would have any relation with your chronic constipation.

Do ask me follow up question.

Thanks and Regards,
Dr.sandipkabra

Brief Answer:
Nausea could be due to orthostatic hypotension or middle ear problem.

Detailed Answer:
Hi,
Thanks for the followup,

There can be a two possibilities for your nausea while bending forward.
It could be due to sudden drop in the blood pressure as a result of change in posture (known as orthostatic hypotension) and that will also lead to fall down blood pressure in intra-cranial arteries in brain which causes your nauseating feeling.

The other reason could be the movement of small hairs i.e. cilia in the semi circular canals of the middle ear while we bend forward from a standing position. It might be either drug induced or some sort of infective component.

You can consult ENT specialist for clinical examination of the ear canal and physician for your blood pressure and its medications.

Thanks
