Brief Answer:
See a surgeon

Detailed Answer:
Hi,
Welcome.
This is Dr Sameer, cardiologist.
The symptoms you are telling could be because of two reasons. One is underlying malignancy & other is Tuberculosis.
I suggest you to go to a general surgeon for further evaluation & management. I guess he will exactly point to the site of abnormality.
Thanks & take care
