Detailed Answer:
Hi.
Noted the history about your son one year and four months old - operated for undescended testis today - advised paracetamol every 4 hours - temperature is now 39.6 in spite of Paracetamol 2 hours ago - not liking to eat anything...
Children of this age do not take anything orally if have fever or anything unusual for them.
If the temperature is not normal, please give cold sponging. This is done by applying tepid/tap water on a cotton clothe to all the body (except for operated area) and allow water to evaporate. Repeat this after the water is evaporated and continue till the temperature fall to normal. This can bring down the temperature to normal within 5-10 minutes. This is very important.
Fever in small children is usually a reflex phenomenon as a response to surgical trauma, but infection may have to be ruled out if the temperature continues to be elevated.
Please consult back to the operating Surgeon or a Pediatrician if the fever continues on.
Continue paracetamol as advised.
What is the dose the Doctor has advised/ what is the concentration of Paracetamol in 5 ml/ what is the weight of the child?
He's wight around 12kg.

As explained below.
Detailed Answer:
Thanks for your feedback.
Nice to know that temperature is under control.
The dose of Paracetamol is children is maximum 15 mg per kg of body weight - 6 - 8 hourly.
With weight of approximately you can give 150 to 180 mg at 8 hourly interval. Now it depends upon the concentration of Paracetamol in 5 ml in the bottle that you have. The dosages can be altered as per the need of the patient.
Once the temperature comes to normal and if the child is playful and drinking/eating well the dosages can be reduced in concentration and/or frequency.
This is the general principle and yet, you have to follow the instruction given by your Doctors as they know the exact/expected condition of the child.
I hope this answers your query.
