I have visited the doctor, will undergo a transvaginal ultrasound and biopsy but am wondering if the biopsy is necessary or there is a high risk for cancer

considering your age and obesity , polymenorrhoea or frequent menses ( 2 weekly) in form of spotting to mild flow with cramping can be most likely to be due to luteal phase defect or anovulation secondary to increased insulin resistance due to obesity. The aim should be to get an ultrasound pelvis done to check for endometrial thickness and also to rule out any PCOS or polycystic ovaries. Endometrial biopsy is being advised not for looking for cancer but to check if your endometrium is in phase with your cycle or not. If the biopsy report for histopath turns out to be proliferative endometrium even on day 21 of your cycle then progesterone deficiency shall be confirmed and you would have to be placed on cyclical progesterones.
So , get the transvaginal ultrasound and biopsy done along with a hormonal profile for PCOS.
