What Causes Frequent Urination And Pain In Penis Glans While Suffering From Prostatitis?

Question: I have tender prostate or prostatitis since last two years..pain in glans, urgent and frequent urination, sometimes urine starts to stream late and very thin stram and with stoppages, sometimes good stream and lot of quantity. PSA levels quite normal and no problem shown by ultrasound and no significan urine left in bladder after voiding. Frequently I get bad smell on my glans and smegma and very mild watery moistness and during these days I suffer above symptoms the most. Life has become very much crippled as I have to get down 2-3 times during my drive to office from home to urinate on these days. Total travel time is only 75 minutes. Since yesterday I have a big swelling on my penis which looks like balanitis. This is second episode, first one was when I was a 6 year old kid I suppose. I think I have pathogens which cause my problem but urine culture does not show them. Because when I took antibiotics, they helped for a few days but later I think due to resistance to them symptoms came back. I have taken Nitrofurantoin, Doxycycline, Livofloxacin, norfolxacin, Septran, but not cured. Please suggest something to take me out of this misery.

Thanks for your query,based on the facts that you have posted it appears that you have Chronic Prostatitis with infection of glans penis and foreskin (Balanoposthitis).

I shall be happy if you can post me few more details to help me in making comments on your problem.
1) Are you diabetic ?
2) Have you ever done Semen Examination and semen culture?
3) Have you done culture of the urethral discharge obtained after doing prostatic massage
4) are you able to retract your foreskin over glans penis?

Please consult qualified Urologist for clinical and digital rectal examination and get following basic tests done to confirm the diagnosis.
1) Semen examination and Culture.
2) Trans Rectal Ultrasound Scanning .(TRUS)
3) Swab test and Culture of Urethral Discharge obtained after prostate massage

Prostatitis take very long time to get cured hence one needs to take medicines for 3-4 months.

As regards your penile problem it could be due to fungal infection of foreskin and glans .This is very common in a person with diabetes ,Get your blood sugar tests done to rule out /confirm the same.

In the mean while start taking following medications
1) Antibiotics like Cefixime twice daily
2) Tamsulosin once daily
3) Diclofenac twice daily
4) Tab Fluconozole twice daily
5) Topical anti fungal jelly for local application.
Later on Switch on to appropriate antibiotics as per culture report .

Thanks Doctor for your reply. I dont have diabetes. I was able to retract foreskin nicely till today morning. Now also I can do it but due to swelling it becomes a bit inconvenient, though not painful. I have not done semen test/culture, TRUS or swab test so far. Local doctor who saw me a few minutes before for swelling on my penis told me it is not balanitis but penile odema caused by may be some insect bite or some allergy. He prescribed cetrizine and urotel. Any advice Dr Sb.

Clinical examination and evaluation by a qualified Urologist will help to diagnose your long standing problem since almost 2 years .

Please get the test done which I have suggested in my reply earlier.

Continue taking medicines I have suggested earlier.

Further treatment will depend upon the results of these tests and final diagnosis.

As regards your penile problem I shall be able to make precise comments on it if you can upload photograph of your penis .
