Brief Answer:
An ear examination is required to proceed further.

Detailed Answer:
Hi,

Thank you for writing back.

1. The images show what seems to be pieces of impacted dry wax and epithelial debris (which indicates excessive earwax build-up that completely blocks the ear canal and prevents the normal shedding of the canal skin). The color is dark black and bluish, which is unusual. I would recommend an otoscopic ear examination to look for any ear wax, ear canal injury or source of an old blood clot.

2. Since the ear canal is narrower in the middle (isthumus), it is unlikely that you can suction all the ear wax out without softening it with wax dissolving ear drops.

3. The lump that you describe was most probably a lymph node. The images of the pinna (external ear show excoriation of the skin near the entrance of the ear canal and the appearance of the rest is normal.

4. A simple ear examination will settle the issue. Do write back with the ear examination findings. Treatment should be relatively simple with wax removal drops, ear cleaning, local skin ointment (combination of an anti-bacterial, anti-fungal and steroid) with oral antibiotics and anti-inflammatory medication.

5. Any hearing problem can also be tested. A Temporal Bone CT, Neck US, FNAC (Fine Needle Aspiration Cytology) can be done to confirm the diagnosis (let your doctor decide),

6. i must emphasize that this is unlikely to be any major health issue.

If you have any more questions I will be available to answer them.

Regards.
