I have read through your query in details.
As you have written about fluid in the sac surrounding the testes, there is a possibility of you having hydrocele and this is increasing in size each day. This can be treated only by surgery and the fluid is taken out alongwith the sac holding it. This is a small surgery and the success rate of this surgery is good. Allowing the hydrocele to grow further might lead to discomfort and sometimes can get painful.
As for the erectile dysfunction, a small hydrocele does not lead to erectile problems. However, with a large hydrocele, a patient at your age might experience discomfort and difficulty to maintain a desired erection essentially due to the large amount of fluid and this can also be psychologically driven.
Studies show that 35%-75% of men with diabetes will experience at least some degree of erectile dysfunction and those with diabetes get erectile dysfunctions 15 years earlier than those who are not diabetic.
