No discharge that I can tell - it does hurt and is little itchy - seems a little bigger today

With your description it can be most probably a fungal infection.
You should consult a gynecologist and get yourself examined and take antifungal drug ( prescription drug) and hydrocortisone cream for local application.
Maintain proper vaginal hygiene, XXXXXXX the area dry, eat less carbohydrate and high protein diet.
Other reasons it may be due to contact dermatitis, herpes, dryness ( due to low estrogen levels) and folliculitis.

Thank you. It seems to have opened up a bit and I am getting dark slightly bloody drainage - not much but some and it seems the size of the hard lump has decreased with the drainage.
Do you think a follow-up appointment with my Dr. is still necessary?
